At your first visit, you will fill out a medical history and patient information sheet to begin your medical record. (*This is important information so feel free to take your time and fill it out as completely as possible.) Every phone call and visit you make will be recorded in this medical record. It is important to us to keep complete and up-to-date data on all our patients to ensure the best health care available.

The information recorded in your medical record is confidential, as is the nature of your visit or anything you discuss with your provider. Our entire staff will protect your confidentiality.

 *Your first visit to our clinic will include a fee to cover our cost in setting you up as a new patient. Subsequent visits will NOT include this additional "new patient" charge.
