An account is established for each family with a running balance. We request that you pay for your health care at the time of the service. Eliminating statements helps keep our fees lower.

Private Insurance. We will file insurance claims for all services unless you request otherwise. Insurance rarely pays the full amount, so we ask that you pay the co-pay and deductible at the time of service. If payment from the insurance company results in an over payment, a refund check will be issued to you.

Self-Pay. Full payment is expected at the time of your visit. A Sliding Fee Scale is available to patients meeting income and family size guidelines. Sliding fee is a discount that is applied to your bill. Please ask the receptionist for further details and a form to complete.

Medicare. The clinic will bill Medicare for services rendered to you. After the payment has been received, we will then submit the remainder of your bill to your secondary insurance. If you don not have secondary insurance, you will be billed for the balance due (if any). See Self-Pay.

Medicaid. Patients on Medicaid or the Children's Health Insurance Program will be required to present their identification card at every visit. The clinic will then bill Medicaid.
